WHO WE ARE
We are a CQC registered organisation and provider of supported living services.
OUR GOALS TO OUR CLIENTS
We will have dedicated project/support workers 24 hours daily to support our clients in exploring all significant areas of independent living and personal development.
The dedicated staff team are fully trained, and have the required skills, knowledge and experience to assist people with disabilities in preparing for the future.
We will provide support for groups of people with similar support needs.
Individuals will be able to reside in any of our accommodation for varying lengths of time, usually for up to two years or more, dependent on their individual needs and the type of supported housing project they live in.
As a service, we will provide the following support services:

- Enabling clients to access their correct benefit entitlement.
- Guidance to budget in an independent manner.
- Support to develop basic practical skills to maintain personal hygiene, including daily hygiene routines.
- Maintaining a clean and safe living environment.
- Food preparation/Healthy eating.
- Linking clients to health services and local community groups such as the GP, Dentist and Optician.
- Linking clients to education, training and employment and as required.
- Reflective support on conduct and behaviour and how to stay safe.
- Socialising and community integration.
- Raising awareness of transport links.
- Raising awareness of local universal services and provider.
- Raising awareness of Banks, Libraries and areas of interest.
ADDITIONAL SUPPORT
- Support with food and grocery shopping (supervised accordingly).
- Support with attending leisure and health activities.
- Support with accessing education, training or employment.
- Support with sexual health and awareness.
- Support with socialising and community integration.
- Support with attending appointments (sign posting and or supervising), money management,
- Administering and safe storage of medication and specific travel training as required. - Skills Development and Empowerment
OUR SERVICE
We provide a small unit accommodation for young people, UASC, and Care Leavers.
We have separate provision for people with challenging behaviour, and in need if personal care.
Our accommodation has the capability to provide personalised services and specialised support with attention to their cultural needs.
Our small unit accommodates at most 3 people in one property. The accommodation will comprises shared housing with shared communal facilities. Our clients will have exclusive use of a fully furnished bedroom with their own personal belonging throughout their stay.
This is further complemented with the assignment of dedicated project/support workers to enable our clients to explore all significant areas of independent living and personal development.
Our dedicated staff team are fully trained, and have the required skills, knowledge and experience to assist people with learning disabilities and vulnerable people in preparing for the future.
Our supported living accommodation with housing support services enable the individuals to move on in a planned way and live independently. The support we provide is at a pace and level appropriate to the needs and aspirations of our clients. Continued support and guidance is provided within all aspects of independent living, assisting and promoting the key aspects and skills required to enable people with learning disability to adapt to change.

This is because effective multi-agency working is an essential tool in achieving best outcomes, giving opportunities for 'joined up thinking', sharing information and striving for the best services that fit the needs of our client's. To achieve excellent and positive outcomes for our clients, regular meetings will be held between key workers and our clients and their families as well as our contracting authorities.

RISK ASSESSMENT
In addition to our client’s support needs, the Referral Co-ordinator will take into account any risk issues. Those who pose a risk to themselves or others due to their behaviour will be placed in supported housing schemes that can manage that individual’s level of risk.
Therefore, a client who posed a high level of risk to themselves or others will not be referred to accommodation that provides a low level of support.
